    Observation: Chlorine sanitizing solution used was not at an acceptable concentration.
    Correction: Adjust the chlorine sanitizing solution to a level of at least 100 parts per million when the pH is at 10 or less and the water temperature is 55°F.

    Observation: Plumbing connections under the warewash area hand sink are leaking. Leaking/disconnected condensate drain in walk in cooler.

    Observation: Less than 50 foot candles of light was noted in the prep counter section of bar area. (fluorescent lamp provided for this purpose was not mounted in position to shed requred light on prep counter.

    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the quaternary ammonis (Steramine brand) chemical sanitizing solutions.
    Correction: Obtain a quaternary ammonium test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Correction: Refinish or replace the indoor ceiling at the area designated so it is: 1. smooth, durable and easily cleanable 2. nonabsorbent, replace light shields (six months to correct these items)

    Observation: Wiping cloths improperly stored between use. Observed used wiping cloths in handsink.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use. Do not use handsink for anything other than handwashing.

    Observation: The water temperature in the manifold servicing the high temperature ware wash machine was below the minimum allowable level of 180ºF. The thermometer installed on the machine does not appear to be working and read 150 degrees continuously. The high temperature recording thermometer that was run through the machine read 145 degrees.
    Correction: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: dipper well and ice machine had mold growth and mineral build up.
    Correction: Clean and sanitize these surfaces for food contact.

    Observation: Water from the handwashing sink at the cooking/salad prep area was measured at a temperature greater than 140°F. No cold water was available due to a broken cold water handle.
    Correction: The water at this sink is too hot to wash hands without scalding. Repair handle for cold water to allow employees to wash hands in warm water.

    Observation: Backflow or backsiphonage prevention device not available on threaded faucet as required by law when a hose under pressure is attached.
