The purpose of today's visit was to conduct a risk factor assessment inspection. Thank you for taking the time to correct any issues during the inspection.
  • Critical: No Bare Hand Contact with RTE Foods (corrected on site)
    Observation: A food employee was observed handling the following ready-to-eat food using their bare hands: ---------------cooked pork while slicing.
    Correction: Except when washing fruits and vegetables, food employees may not contact ready-to-eat foods using their bare hands. Employees shall use suitable utensils such as deli tissue, spatulas, tongs, or clean disposable gloves to handle all ready-to-eat foods. DISCARDED.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device:
    1) Crab sticks at 46F in Hoshizaki Sushi Cooler right,
    2) Fish cake at 46F in Turbo Air 2DR prep cooler

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. ITEM # 1 WAS DISCARDED SINCE SUSHI CHEF STATED CRAB STICKS WERE THAWED OVERNIGHT AT ROOM TEMPERATURE. ITEM # 2 WAS RETURNED TO INSIDE COOLER HOLDING BELOW 41F.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp (corrected on site)
    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution.
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine. UNIT WAS PRIMED AND CHLORINE WAS CHANGED AND THEN OBSERVED 50 PPM CHLORINE. ALL RECENTLY WASHED RUN THROUGH MACHINE AGAIN.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs.
    Observation: Surfaces of ice bins and enclosed components of equipment were observed in need of cleaning.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located at the sushi case is blocked, preventing access by employees for easy handwashing.-------------Observed a metal container sitting in handsink and water was turned off at handsink.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use. METAL CONTAINER RELOCATED AND WATER TURNED ON.

    Correction: The identity of the source of shellstock (oysters, clams, mussels) that are sold or served shall be maintained by retaining shellstock tags or labels for 90 calendar days from the date when the last shellstock from the container is sold or served using one of the following methods: 1) an approved record keeping system that keeps the tags or labels in chronological order correlated to the date when the last shellstock from a container are sold or served, 2) preserving source identification by using a record keeping system, and 3) ensuring that shellstock from one tagged or labeled container are not commingled with shellstock from another container with different certification numbers, different harvest dates, or different growing areas as identified on the tag or label before being ordered by the consumer.

    Observation: The following food items that are not easily identified by appearance were observed without a label: sauce, wine, spices.
    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc. EMPLOYEE LABELED FOOD CONTAINERS.

    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

    Observation: The handwashing facility located at the sushi station is blocked, preventing access by employees for easy handwashing.
    Correction: A handwashing sink shall be maintained so that it is accessible at all times for employee use. Articles blocking the hand sink were removed.

    Observation: Observed carpet in a room where there is food storage in a refrigeration unit.
    Correction: Remove the carpet and replace with a floor that is smooth and easily cleanable.

The manager will maintain a log for products under Time as a public health control and monitor the temperature of the tofu.
Dishmachine is a chemical sanitizer.

    Observation: Open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.
    Correction: Food employees may have beverages in the kitchen areas as long as they are covered and consumed through a straw and stored in a manner that will prevent contamination to food, equipment, utensils, linens, and single service items.

    Observation: Tags missing from the molluscan shellfish containers.

    Observation: Raw animal food stored over ready-to-eat food in the refrigeration unit. OBSERVED RAW SHELLED EGGS STORED ABOVE CARROTS IN THE WALK IN COOLER.
    Correction: Raw foods like poultry, beef, pork, seafood, and eggs shall be stored properly inside refrigerators and freezers to prevent cross contamination. Raw foods shall be stored on shelving units BELOW or SEPARATED from all cooked foods, commercially processed foods, washed produce, leftover foods, etc. MANAGER REMOVED CARROTS FROM THE BOTTOM SHELF AND PLACED RAW SHELLED EGGS BELOW CARROTS.

    Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: Beans at the buffet are not covered and the raw marinating beef in the walk in cooler is not covered.
    Correction: Foods shall be protected from contamination by being stored in packages, covered containers or wrappings unless being cooled. MANAGER COVERED FOOD.

A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:

    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: knives observed to be stored in between cooling units.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.     Observation: The following farm-raised fish (SALMON) served raw or partially cooked were observed without a written statement from the supplier stating that the fish were raised and fed according to 3-402.11(B)(3).
    Correction: Contact your fish supplier and ask for a written statement stating that the fish were raised in net pens if in open water OR land-based operations AND fed formulated feed that contains no live parasites. FAX a copy of this letter to your inspector and keep this statement on site at the food establishment for 90 days beyond the time of service of the fish.
  • Critical: Consumer Advisory Required for Raw or Undercooked Animal Food
    Observation: A review of the menu with the foodservice operator indicates that there is no consumer advisory for the following food item(s) that may be served raw and/or undercooked: THE TAKEOUT MENU DOES NOT PROVIDE A CONSUMER ADVISORY ON THE SUSHI, SASHIMI, AND SUSHI A LA CARTE PORTION.
    Correction: If an animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw or undercooked the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means.
