    Observation: Unwrapped or uncovered food in the two warming cabinets and hot holding units, also bottom layer of RTE items above the prep line that are not protected by a plastic gaurd are uncovered and exposed to possible contamination.
    Correction: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
  • Ventilation Systems - Cleaning Ventilation Systems - Nuisance and Discharge Prohibition (repeated violation)
    Observation: Vent filters and hood system observed with an accumulation of grease and debris and are not being maintained in a clean condition. Also no inspection/cleaning sticker provided on the unit.
    Correction: Maintain hood system and vent filters in a clean condition.

Little effort put in to correct and fix items. Stated taco bell policy on food items uncovered, explained state regulations supersede restaurant policy. Explained that it no attempt to correct items within ten days of this inspection a possible notice of alleged violation would be issued that can result in legal proceedings. PIC stated that all items were manager decision, explained that since she was placed as a PIC (person in charge) she is the responsible party at time of inspection. No efforts made to correct still uncorrected items at time of inspection.

    Observation: Unwrapped or uncovered food in the two warming cabinets and hot holding units, also bottom layer of RTE items above the prep line that are not protected by a plastic gaurd are uncovered and exposed to possible contamination.
    Correction: Protect food from cross contamination by storing food in packages, covered containers, or wrappings.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: (1) the floor of the walk in freezer was observed with food debris, (2) Food debris observed in the bottom of the two door delfield next to fryer station, (3) accumulation of oil and grease observed down the sides, underneath and inside of the fryer.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Critical: Backflow Prevention, Air Gap* (repeated violation)
    Observation: An air gap between the water supply outlet and the flood rim level is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system. Drains from drink machines observed inserted below the flood level of the drain pipe under the drink system at the drive in window.
    Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

    Observation: Food stored on the floor or food stored less than 6" above the floor. Two boxes observed on the floor of the walkin freezer.;Elevate food storage onto approved shelving with minimum 6" legs or casters.
  • Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
    Observation: EHS observed cardboard being used at fryer station as a cover on the table to absorb oil.
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils

    Correction: The handwash facility identified above is to be used for washing hands only. Clean and sanitize sink prior to using again.

    Observation: No disposable towels were provided at the hand washing lavatory in the rear.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

NOTES:
- Observed light cleaning needed underneath units at food prep areas. Also, observed each trash can filled over with trash. Ensure trash is changed often to prevent the harborage of flies.
- Observed frequent hand washing and proper glove use.
- Hats/hairnets worn during food service operations.
- Hand sink in rear located at prep table. Ensure that during food preparation, the hand sink is not used or install a splashboard to prevent possible contamination to prepared foods.
- All staff possess current Food Handler card.

  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the one deep fryer has accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The dining area floor and tables noted in need of cleaning.
    Correction: A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner. Ensure dining area is cleaned during low business hours.

Second pre-opening inspection notes:
- Observed no light bulb in the walk-in freezer.
- Two hand sinks in the kitchen need a hand washing sign. EcoLab to arrive at facility today to provide proper hand washing signs. EHS gave operator two hand washing signs to post as an option if EcoLab does not arrive.
- Hand washing sink located behind ordering station may need a splashboard installed. Per operator, all food near the hand washing sink will be wrapped and not exposed to possible contamination. Suggested to operator that by installing a splashboard, any possible contamination by hand washing may be eliminated.
- Ensure all employees obtain Food Handler cards prior to employment.
- Permitted to operate as a food service facility.

  • Lighting, Intensity (repeated violation)
    Observation: Walk in freezer observed with no light bulb.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.

    Observation: Employees are not properly trained in food safety as it relates to their assigned duties.
    Correction: Train all employees in food safety as it relates to their assigned duties. Ensure ALL staff possess a current Food Handler card issued by Western Tidewater Health District prior to working with food.
  • Temperature Measuring Devices
    Observation: There was no temperature measuring device located in the cold holding units.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Handwashing Signage/Handwashing Facilities
    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees.
    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.
  • Lighting, Intensity
    Observation: Walk in freezer observed with no light bulb.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.
