The purpose of this visit was to conduct a follow-up inspection on both victory 2dr coolers. Both coolers have an ambient air temperature of 41F and below and are able to keep potentially hazardous food at 41F and below.

  • Gloves/Use Limitation/No Re-Use/Use For Only One Task (corrected on site)
    Observation: Single-use gloves worn during multi-tasked food preparation.
    Correction: If used, single-use gloves shall be used for only ONE task and used for no other purposes. The single-use gloves shall be discarded when damaged or soiled or when interruptions occur in the operation. EHS went over glove usage with CFM and food employees. FOod employees discarded gloves, washed hands, and changed into new gloves
  • Critical: PHF(TCS) Cooled from 135°F to 41°F within 6 Hours Total (corrected on site)
    Observation: The following cooked potentially hazardous foods are not being adequately cooled from 135°F to 70°F within 2 hours and to 41°F within a total of 6 hours: red curry 47F and green curry 44F both in victory 2dr cooler #1 right side
    Correction: Potentially hazardous foods shall be cooled from 135°F to 70°F within 2 hours and to 41°F within a total of 6 hours to prevent the growth of harmful bacteria. CFM discarded curries

    Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at front cashier bar handwashing sink used by food employees.
    Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.

    Observation: The concentration of the chlorine solution was measured at over 200ppm. This concentration exceeds the approved limits for use with food-contact surfaces of equipment and utensils in a foodservice establishment.
    Correction: Maintain the concentration of chlorine solution at 50-200ppm. Verify concentration using the appropriate test kit. Food employee reset solution to 100ppm

    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: raw fish over cooked noodles in Victory 2dr upright cooler #2 left
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. CFM placed fish at the bottom shelf
  • Utensils That are In-Use / Between-Use Storage
    Observation: Dispensing and/or in-use utensils improperly stored between use as follows: utensils stored in between wall and equipment
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently.

    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: garlic and oil mix 76F and bean sprouts 75F found at room temp
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. garlic and oil mix was put in a cooler and ice was added to bean sprouts
  • Food Temp Meas Device, Calibrated
    Observation: The food temperature measuring device was found out of calibration in the range of use.
    Correction: Food thermometers shall be calibrated in accordance with manufacturer's specifications as necessary to ensure their accuracy. It is suggested that food thermometers be calibrated at least once a week in a cup completely filled with ice and cold water to a temperature of 32°F or calibrated in a pot of boiling water at 212°F.
  • Handwashing Sink / Accessible at All Times (corrected on site)
    Observation: The handwashing facility located by the cashier is blocked, preventing access by employees for easy handwashing.
    Correction: A hand washing sink shall be maintained so that it is accessible at all times for employee use.

    Observation: The following refrigerated, ready-to-eat, potentially hazardous food that is prepared on site is not used or discarded within 24 hours of preparation and was not observed to be date marked:observed in 2dr Victory: sticky rice, beef curry.
    Correction: Refrigerated, ready-to-eat, potentially hazardous foods that are prepared on site and kept for more than 24 hours cannot have a shelf-life of more than 7 days, including the day of preparation. These food items must be marked with either the "consume-by" date and/or "preparation" date, color-coded labels, or another marking system/method that effectively indicates shelf-life that is clearly understood and properly used by the employees.

Manager cleaned the ice bin during the inspection.

    Correction: Unless a food item can be easily recognized, all foods and ingredients removed from their original containers and stored in working containers shall be labeled using the common name of the food. For example: cooking oils, salt, sugar, flour, spices, herbs, etc. (each h food container was labeled accordingly)
  • Wiping Cloths/Use Limitation/Wet Towel Storage
    Observation: Wet wiping cloths used for wiping counters and other equipment surfaces were observed stored on the counters and other work stations.
    Correction: Wet wiping cloths are to be used for wiping counters and other equipment surfaces and are to be held in a chemical sanitizer solution at the appropriate concentration between uses. after each use, soak wet wiping towels in sanitizing solution.
  • Critical: Cold Holding of Potentially Hazardous Food at 41°F or less (corrected on site)
    Observation: The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: cooked duck =49-F, chicken on stick=45-F, shrimp rolls=47-F.
    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below unless the permit holder is using "time as public health control" as specified under 3-501.19 to limit bacteria growth. (Items relocated to freezer for cooling)
  • Temp Meas Devices for Ambient Air Location (corrected on site)
    Observation: The temperature measuring device in the following mechanically refrigerated unit was not properly located in the warmest part of the unit: cookline refrigerators.
    Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.
  • Food Temperature Measuring Device Required & Readily Accessible
    Observation: The certified food manager could not provide a good working food temperature measuring device.
    Correction: A metal-stemmed probe food thermometer that is in good working condition shall be provided and readily accessible for use in ensuring attainment and maintenance of food temperatures.
  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp
    Observation: When tested, the chlorine-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the warewashing machine with a concentration of zero ppm total chlorine.
    Correction: A chlorine-based sanitizing solution shall have a minimum concentration of at least 50 to100 ppm total chlorine when tested using the appropriate test kit.
  • Sanitizing Solution Test Kit Accurate & Used
    Observation: The operator is not testing the chemical sanitizing solution to ensure proper concentration of the solution.
    Correction: The operator shall be monitoring the concentration of the sanitizing solution using a test kit or other device.
  • Critical: Food-Contact Surface Sanitized before Use after Cleaning
    Observation: The food-contact surfaces of the following equipment were not observed sanitized: utensils washed at the warewashing machine.
    Correction: All utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning.
  • Critical: Sanitize/Chemical/Manual & Mechanical/Concentration & Exposure Time
    Observation: Due to improper operation of the low temperature chemical sanitizing mechanical warewashing machine, equipment food-contact surfaces and utensils are not sanitized after cleaning. (see 4-501.114)
    Correction: Equipment food-contact surfaces and utensils are therefore not adequately sanitized after cleaning due to improper function of the warewashing machine.
  • Outer Openings, Protected / Screen Requirements
    Observation: Openings to the exterior of the building are present along the base of the back door to the kitchen.
    Correction: Fill or close holes and other gaps along floors, walls, and ceilings to protect against the entry of insects and rodents. Install a weather stripping at the base of the back door to seal the existing gap.
  • Maintaining Premises Free of Unnecessary Items and Unused Equipment
    Observation: Observed that items that are unnecessary to the operation or maintenance of the establishment are being stored on the premises.
    Correction: Remove unnecessary items, including equipment which is no longer in use, from the premises to allow for regular and effective cleaning and to prevent harborage conditions.

    Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name. An unmarked spray bottle containing cleaning chemical solution was n to labeled to identify contents.
    Correction: Label all working containers of poisonous and toxic materials, including cleaners and sanitizers, with a common name to prevent the dangerous misuse of these materials.

    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.

    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: clean shelves and area at the back, food debris on floor.
    Correction: Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
