    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a chemical test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Observation: Ice buildup in Kenmore fridge.

    Correction: Alter or replace this equipment so that the filters can be easily removed for cleaning and replacement. Clean hood system and have servicer post current sticker on hoods after cleaning.

Product in cooler unit to placed on ice to hold at proper temp of 41 degrees and below. Informed manager to recheck glass stacking at bar and wait station area. Copy of Consumer Advisory information given to manager. Facility recommended for permit. Copy of food permit, copy of receipt and copy of application given to manager.

    Observation: Animal food such as beef, eggs, fish, lamb, milk, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in Ready-To-Eat form or as an ingredient in another Ready-To-Eat Food..
    Correction: The Permit Holder shall inform Consumers of the significantly increased Risk of consuming such Foods by way of a Disclosure and Reminder using brochures, deli case or menu advisories, label statements, table tents, placards, or other effective written means. Add additional information to consuner advisory that is on lunch and dinner menus.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Door on cooler, doors on 3 door prep unit in need of repair.
    Correction: Repair the doors on cooler and 3 door prep unit to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the doors, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Non-Food Contact Surfaces
    Observation: Door gasket on GE freezer in need of cleaning due to buildup in gasket.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of buildup that become encrusted or attract insects and rodents.

    Observation: Observed unwrapped spoons improperly stored in plastic containers with handles up.
    Correction: To prevent contamination of the food and lip-contact surface, provide a means of storage where handles are stored upward inside containers.

Spoke with certified food managers concerning inspection. Certified food manager called servicer during inspection concerning dishwasher. Continue to use three compartment sink until dishwasher is properly working. Dining area and rest rooms are clean. Facility recommended for permit. Permit, copy of receipt and copy of application issued.
