3-304.14(B)(1) - wet wiping cloths on counter Cloths in use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a specified concentration.
3-305.11(A)(3) - Corrected During Inspection Food stored on the floor and/or food stored less than 6 inches off the floor. Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
4-204.112(A) - The temperature measuring device in the True undercounter refrigerators were not properly located in the warmest part of the unit: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.
4-301.11 - The takeout display refrigerator thermometer is reading 42f and the foods in it are over 41f Equipment for cooling and heating food, and holding cold and hot food shall operate properly and hold cold potentially hazardous foods at 41f or lower. ADJUST THE UNIT TO LOWER THE TEMPERATURE
6-301.13 - Observed that hand soap dispenser has been installed at the three vat sink Remove the soap. Use only handwashing sinks for handwashing
6-301.14 - Corrected During Inspection Observed that sign or poster that notifies food employees to wash their hands is not provided at all handwashing sinks used by food employees. Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands.

6-501.16 - Corrected During Inspection Observed that mops are improperly stored between use. Store mops in a position that allows them to air dry without soiling walls, equipment, or supplies.
Comments:
This is a routine inspection at this coffee shop. There are three CFMs. No Consumer Advisory, very limited menu. Packaged foods, pastries and beverages only. Pest control monthly. No grease trap. Water heater installed on shelf above mop sink.

The purpose of today's visit was to conduct a critical procedures inspector. If there are any questions please contact the EHS inspector. Thank you. Notes: Hang the mop up so it can air dry. Employees need hairs restaints.

Violations:
3-501.16(A)(2)(a) - Critical The following food item(s) were observed cold holding at improper temperatures using a calibrated food temperature measuring device: milk and soy milk in front units Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41F or below. The units should be adjusted to hold temperature during busy hours.
4-501.114(C) - Corrected During InspectionCritical Quaternary ammonium compound sanitizing solution used was not at an acceptable concentration. The observed concentration was 400ppm. A quaternary ammonium compound solution shall have a minimum temperature of 75F, a concentration as specified under 7-204.11 and as indicated by the manufacturer's use directions included in the labeling.
43.1-3-1(a) - The establishment has not posted the permit to operate in a conspicuous location. No person shall operate a food establishment unless that person possesses a valid permit issued by the Director. Only a person who complies with the requirements of this Chapter shall be entitled to receive or retain such a permit. Permits are not transferable from one owner to another. A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
