Chapter 2-402.11 Effectiveness.
(A) Except as provided in ¶ (B), FOOD EMPLOYEES shall wear hair restraints such as hats,
hair coverings or nets, beard restraints, and clothing that covers body hair, that are
designed and worn to effectively keep their hair from contacting exposed FOOD; clean
EQUIPMENT, UTENSILS, and LINENS; and unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES.
(B) This section does not apply to FOOD EMPLOYEES such as counter staff who only serve
BEVERAGES and wrapped or PACKAGED FOODS, hostesses, and wait staff if they present a
minimal RISK of contaminating exposed FOOD; clean EQUIPMENT, UTENSILS, and LINENS; and unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES.

Our Department received a complaint from a customer who went thru the drive-thru during the lunch hour on 3-9-09 and found what she feels was
a short hair in the box containing her sandwich. During this visit, all employees working on the prep line were wearing visors and had long
hair tied back.
The Manger on duty feels that the material was a hair like piece of cardboard often found inside the box from the manufacturing of the box. I
was able to observe one of these cardboard strands. It does appear hair like.

Our Department received an anonymous complaint from a customer that stated that there was no water in the restrooms.
At the time of this visit, water was available in both restrooms. The manager on duty states that the water did go out earlier this a.m. She
feels that the problem may have been breaker related. Service was called to check it out.

**CDC Risk Factor Violation 14 F**:
• Some small utensils in clean storage were not clean to sight and touch. They were removed for rewashing during the inspection.
4-601.11 Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils.
(A) EQUIPMENT FOOD-CONTACT SURFACES and UTENSILS shall be clean to sight and touch.
(B) The FOOD-CONTACT SURFACES of cooking EQUIPMENT and pans shall be kept free of
encrusted grease deposits and other soil accumulations.
(C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of
dust, dirt, FOOD residue, and other debris.
