- Observed at time of inspection menu is provided with asterisked disclaimer and reminder statement. However menu items that can be cooked to order or served raw are not identified with an asterisk. Also consumer advisory print is smaller then the print describing the menu items. Correct by enlarging print for consumer advisory and providing asterisks for menu items that can be cooked to order or ordered raw. Correct immediately a follow up will occur at the next routine inspection.

- Observed at time of inspection damp cloth stored onto of lettuce in prep cooler. Food should only be coming in contact with surfaces that are approved for food contact. Damp cloths or paper towel can contribute to the spread of harmful bacteria. Corrected at time of inspection by removing cloths and disposing of lettuce.
