Violation descriptions and comments

5-13-11, Trailer did not have license plate, asked they call in the plate number when they have the trailer tagged. Asked they get the FH cards in place and to keep them in the mobile trailer. They had hand soap and towels, but I asked that they have way to keep them at the sink. Also asked that they label all the sinks for clairity of use. Approved permit. WCCHD to mail permit.
