Violation descriptions and comments

An employee in the dish washing area was observed handling dirty then clean dishes without washing their hands in between.
An employee in the dish washing area was observed wiping their hands on their pants then continued to handle clean dishes. A kitchen employee was observed scooping ice with a cup at the front service area soda fountain.
The hot water was turned off on the hand sink located next to the three compartment sink.
Paper towels were not available at the front and back kitchen hand sinks.
Paint containers were stored above single service containers in the back kitchen storage cabinet. Canned heat containers were stored next to single service containers under the cash register.
Three spray bottles of chemicals stored above the three compartment sink were unlabeled as to their contents.
Non food grade containers were used to stored corn bread on the preparation table located next to the kitchen refrigerator.
A non commercial freezer and refrigerator were used to store food products in the kitchen
Chlorine test strips for the chlorine dish machine were not provided upon inspection.
Multiple tongs were stored at knee level on the side of the pizza ovens. Multiple utensils stored in the container next to the kitchen refrigerator were stored in opposing direction. The handle of the scoop stored in the ice container at the front service soda fountain was stored touching the product.
Multiple single service containers were stored on the floor in the back kitchen storage cabinet.
Multiple holes were present in the walls located in the dish washing area. The ceiling tile located above the kitchen stand up freezers was missing.
The lights above the three compartment sink were uncovered.
A ventilation hood was not present above the fryer in the kitchen.
A cell phone was stored on top of single service containers in the back kitchen storage cabinet.

Moldy lettuce was stored with wholesome lettuce in the standing refrigerator. Raw catfish and raw shrimp were stored above ready to eat lettuce, cabbage, and cheese.
An unapproved employee beverage was stored on the preparation table behind the three compartment sink.
