In food establishments, there shall be adequate sewage and waste water disposal facilities that comply with all the requirements of the plumbing section of the Municipal Code of Chicago. THE DRAIN LINE NEXT TO THE HOT WATER HEATER HEATER LEAKS AND THE SODA GUNS HAVE NO DRAIN LINES CONNECTED. REPAIR DRAINAGE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. NO PEST CONTROL LOG BOOK ON PREMISES. PESTPROOF THE EAST SIDE FRONT DOOR WHERE THE DOORS MEET.
Equipment and utensils should get proper exposure to the sanitizing solution during the rinse cycle. Bactericidal treatment shall consist of exposure of all dish and utensil surfaces to a rinse of clean water at a temperature of not less than 180F. THE HIGH TEMPERATURE DISH MACHINE HAS A 175 F FINAL RINSE, ADJUST THE FINAL RINSE TEMPERATURE TO 180 F.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS THROUGHOUT THE BASEMENT AND INSIDE THE WALK IN COOLERS AND FREEZERS ARE UNSEALED CONCRETE, SEAL SAID FLOORS SO THEY ARE SMOOTH AND SANITARY.
The walls and ceilings shall be in good repair and easily cleaned. THE BASEMENT STORAGE WHICH HAS ICE AND SODA MACHINES AND DRY FOOD STORAGE HAS ROUGH, UNSEALED CONCRETE WALLS AND UNFINISHED DRY WALL CEILINGS. SMOOTH OUT THE WALLS AND SEAL THE WALLS AND CEILINGS SO THEY ARE SMOOTH AND EASY TO CLEAN.
All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces. THE BASEMENT STORAGE AREAS ARE INADEQUATELY LIT, INSTALL ADDITIONAL LIGHTING TO AID IN INSPECTING AND CLEANING THE BASEMENT.

AN UP TO DATE PEST CONTROL LOG BOOK IS ON PREMISES. THE EAST SIDE FRONT DOOR IS NOW PESTPROOF WHERE THE DOORS MEET.
THE FLOORS THROUGHOUT THE BASEMENT AND INSIDE THE WALK IN COOLERS AND FREEZERS ARE NOW SEALED CONCRETE.
THE WALLS AND CEILINGS SO THEY ARE SMOOTH AND EASY TO CLEAN IN THE BASEMENT.
ADDITIONAL LIGHTING TO AID IN INSPECTING AND CLEANING THE BASEMENT HAS BEEN INSTALLED.

THE DISH MACHINE HAS A LIME BUILDUP INSIDE, THE CAN OPENER AND MICROWAVE OVEN ARE DIRTY; WASH, RINSE AND SANITIZE ALL OF THE FOOD AND NON-FOOD UTENSILS AND EQUIPMENT UNTIL IT IS CLEAN, SANITARY AND MAINTAINED ON A DAILY BASIS.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE KITCHEN FLOOR CORNERS ARE DIRTY, CLEAN THE FLOORS IN DETAIL.
THE HOT WATER HEATER HAS A SMALL LEAK, REPAIR THE LEAK AND CLEAN UP THE FLOOR UNDERNEATH.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE WALK IN COOLER HAS A DIRTY COIL. CLEAN THE COOLER COIL.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. THE ICE SCOOP HOLDER IN THE BASEMENT IS DIRTY; WASH, RINSE AND SANITIZE ALL OF THE FOOD AND NON-FOOD UTENSILS AND EQUIPMENT UNTIL IT IS CLEAN, SANITARY AND MAINTAINED ON A DAILY BASIS.
