A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. No one on premises with a city sanitation certificate while handling/serving P.H.F.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. All cutting boards with deep/dark grooves must be sanded/bleached or replaced-to make surfaces smooth/easily cleanable. Must fix replace missing splashguard between prepsink & 3-comp sink.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors under shelvings in walk-in cooler, thruout dry storage room, under 3-comp sink & around grease traps need detailed cleaning.
The walls and ceilings shall be in good repair and easily cleaned. Dusty condensers/compressors of coolers, esp. the walk-in cooler must be kept clean.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Missing light shields in rear prep/dishroom area must be replaced, or use shatter-proof bulbs.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. Soda cup lids & disposable cups/glasses must be stored upside down with a barrier between the lip of cups/lids & countertop.

FOUND FOLLOWING: 7-ON WALL & SIGNAGE BOARD ABOVE THE FRONT HANDSINK, 6-ON EXTERIOR WALLS OF WALK-IN COOLER, 7-ON THE HOOD IN REAR ROOM, 8-ON WALLS ABOVE THE 3-COMP SINK. INSTRUCTED TO CLEAN/SANITIZE ALL AFFECTED AREAS. All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.
(7-38-030) BATHROOM NOT PORPERLY MAINTAINED. FOUND OLD EQUIPMENT, PRE-PACKAGED FOOD ON A SHELF, EMPLOYEE BELONGINGS & OTHE UNNECESSARY ARTICLES STORED IN EMPLOYEE BATHROOM, WHERE YOU HAVE TO STEP OVER SOME OF THESE THINGS TO ACCESS THE TOILET BOWL OR HANDSINK. INSTRUCTED TO REMOVE ALL THESE ARTICLES & MAINTAIN. When toilet and lavatory facilities are provided for the patrons of food establishments, such facilities shall be adequate in number, convenient, accessible, properly designed, and installed according to the municipal code.
WATER ON PIPES UNDER THE PREP SINK & REAR HANDSINK MUST BE FIXED. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
GASKETS OF COOLER DOORS, CONDENSER OF WALK-IN COOLER, EXTERIOR OF SINKS, CAN OPENER, INTERIOR OF ALL GARBAGE BINS MUST BE KEPT CLEAN. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
