UPON INSPECTION FOUND MEN'S TOILET ROOM OUT OF ORDED. PREMISES IS A TAVERN THAT REQUIRES MEN'S AND WOMEN'S FACILITIES. WORK IS BEING DONE IN THE MEN'S ROOM. MUST HAVE BOTH TOILET ROOMS WORKING WHEN THE TAVERN IS OPEN FOR BUSINESS. SERIOUS CITATION GIVEN 7-38-030
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. THE FRONT DOOR NOT TIGHT FITTING OR INSECT OR RODENT PROOFED. HAS A ONE FOURTH GAP AT THE BOTTOM OF THE DOORS. NO PEST CONTROL RECIEPT OR LOG BOOK FROM A LICENSED PEST CONTROL COMPANY. INSTRUCTED TO MAKE THE FRONT DOOR TIGHT AND RODENT PROOF IT AND PROVIDE RECIEPT AND LOG BOOK.
REPAIR THE FLOOR IN THE TOILET ROOMS THEY ARE UNSAFE. CLEAN ALL THE FLOORS IN THE BASEMENT AND 1ST FLOOR.
The walls and ceilings shall be in good repair and easily cleaned. SEAL AND REPAIR ALL THE WALLS THROUHGOUT PREMISES. REMOVE THE HANGING PAINT AND PAINT SAME
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. CLEAN THE BAR AREA, CABINETS, COUNTER,SHELVES AND SINKS.

VIOLATIONS CORRECTED. PREMISES NOW HAS TERMINIX PEST CONTROL THAT WILL COME ONCE A MONTH TO SERVICE THIS ESTABLISHMENT. ALSO THE FRONT DOOR IS TIGHT FITTING AND RODENT PROOFED.
THE MEN'S AND WOMEN'S TOILET ROOMS ARE NOW NEWLY REMODELED WITH ALL NEW FIXTURES, WALLS, CEILING AND FLOORS.
