THE BASE OF THE FAUCET IS LEAKING AT THE 3 COMPARTMENT SINK. NEEDS TO REPAIR
All food shall be protected from contamination and the elements, and so shall all food equipment, containers, utensils, food contact surfaces and devices, and vehicles. FOUND BLACK MOLD IN THE UPPER PART OF THE ICE MACHINE WHERE THE ICE IS MADE ON TRAY AND INNER WALLS OF MACHINE. INST TO DISCONNECT AND CALL FOR SERVICE. TAG UNIT. All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FOUND 1/4 INCH GAP AT THE BOTTOM OF DOOR LOCATED AT 5707 N. CLARK. NEEDS TO BE TIGHT FITTING TO PREVENT RODENT OR ANY TYPE OF INSECT.CITATION ISSUE
All food not stored in the original container shall be stored in properly labeled containers. ALL CONTAINER IN THE BAKERY AREA NEEDS AND PACKAGE FOOD ITEMS NEED TO HAVE NAME COMPANY INGS. ALL CONTINERS IN STORAGE NEEDS LABLES AND DATE OF ALL ITEMS.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REMOVE ALL MILK CRATE AND SODA CRATE THAT ARE BEING USED AS STORAGE SHELVES NEEDS TO BE ABLE TO CLEAN AND STORE PROPERLY. THE FOLLOWING NOT CLEAN- OBSERVED CALCIUM BUILD UP INSIDE ICE MACHINE AND DUST BUILD UP ON PREP AREA CONDENSERS. MUST CLEAN AND MAINTAIN EQUIPMENT.
FLOOR BASE AT WALL CORNERS IN POOR REPAIR, PEELING OFF WALL (ADJCENT TO DISHWASH 3-COMPARTMENT SINK). MUST REATTACH FLOOR BASE TO WALLS/SEAL GAPS AT SAID.
