Found inadequate waste water disposal i.e. waste water coming up from floor drain onto floor when a compartment at the 3 comp sink is filled then released and waste water not properly draining out from floor drain beneath the 1 comp sink at the kitchen-prep area.
Unused kitchen equipments from previous business must be removed from premises.
Found no city of chicago sanitation certificate or valid documentation on premises during this inspection.
Found front door to the food establishment not insect/rodent proofed must be sealed all accross the bottom of said door and provide,maintain a map locator inside the log book for identifying pest control services.
Found black slimy substance on interior water components of the ice machine.
