All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed accumilated dust build up on prep coolers compressors/condensors. Instructed manager to clean and maintain all unclean equipment.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floor base in poor repair at 1st floor prep cookline by chest freezer and in employees Womens washroom. Must repair and maintain floor base.
The walls and ceilings shall be in good repair and easily cleaned. Must clean unclean ceiling light covers at 1st floor prep area and maintain.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Electrical and Hot water tank room cluttered throughout. Must elevate all items 6' off floor or remove items to prevent potential pest harborage.

OBSERVED THICK BLACK SLIME/DIRT BUILD UP INSIDE ICE MACHINE DRIPPING INTO ICE (READY TO EAT FOOD). INSTRUCTED MANAGER TO CLEAN AND SANITIZE INTERIOR OF ICE MACHINE. ALL ICE VOLUNTARILY DISCARDED AT THIS TIME. SERIOUS VIOLATION. CITATION ISSUED. TICKET #HOOOO63340 18.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED FOOD DEBRIS/FOOD SPILL AT FLOOR UNDER SHELF RACKS AT BASMENT DRY STORAGE ROOM. MUST DETAIL CLEAN FLOOR UNDER AND AROUND ALL SHELF RACKS.
The walls and ceilings shall be in good repair and easily cleaned. 2ND FLOOR MENS WASHROOM WALLS AND BASEMENT DRY STORAGE ROOM WALLS NOT CLEAN. MUST CLEAN AND SANITIZE ALL UNCLEAN WALLS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. POT WASH 3-COMPARTMENT SINK PIPE LEAKING UNDER MIDDLE COMPARTMENT. MUST REPAIR AND MAINTAIN SINK.
