The walls and ceilings shall be in good repair and easily cleaned. Found ceiling not clean at 2nd floor preparation areas.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily Found containers not stored properly at 2nd floor preparation areas.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. Found wash cloths not stored properly.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. BASEMENT 3-COMPARTMENT SINK PIPE LEAKING UNDER RIGHT SIDE COMPARTMENT. MUST REPAIR AND MAINTAIN PIPE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED DIRT/FOOD BUILD UP INSIDE BAR AND PREP AREA FLOOR DRAINS. MUST CLEAN AND MAINTAIN INTERIOR OF ALL UNCLEAN FLOOR DRAINS.

FOUND FRUIT FLY ACTIVITY AT DISHWASHING AREAS MAIN FKITCHEN. (OBSERVED ABOUT 20 FRUIT FLIES)
