The meals bread , water , the meal taxs. and what ever tipping are always better in Europe ,just ask any European or snotty traveler.
Folks are only comfortable with what they are use to.

Eating out in small towns in the American midwest is ridiculously inexpensive. The small family owned diners and kitchens are often less expensive than fast food. My family had a $25 meal at some little nondescript Indiana pizza parlor between Indy and South Bend that would have cost $150+ in NYC or Chicago. I couldn't believe it when I got the check it was a large meal with ice cream desserts for the cost of a cocktail.

I have found that major European cities I have visited in the last 5 years or so (London, Amsterdam, Dublin, Brussels) are every bit as expensive as NYC and often more so. I have never found anything in Europe remotely as cheap as rural America, although maybe I just haven't gotten off the beaten path far enough in the right countries. I am pretty sure there is no place in the UK or Ireland as inexpensive as the American midwest, though.

America has all kinds of problems with obesity, but the subset of people who frequently enjoy fine dining have next to zero impact on the obesity rates. $10/family chicken buckets and triple bacon cheeseburger combos along with lots of sugary drinks are much bigger contributors than bottles of wine and 5 course meals.
