One night, my wife and I were going from Louisville, KY to our hotel in Newport, KY, as we were going to be seeing a Reds game that night. Upon driving in, Google had us go across the river into Ohio, go across the Cincinnati riverfront, then head back south across the river back to Kentucky.

We walked back into Ohio for the game that night. Upon leaving, we went back over another bridge, and ended up in Kenton County, and had to walk across it (and another river) to get back to our hotel in Newport (Campbell County).

I do have pics of this fantastic evening that I may post.

The Washington - Oregon state line is an absolut straight line from the Idaho Border to the Columbia River. And the Columbia river is pretty straight too, except for a North - South jot, between Longview Washington and Portland Oregon.

The Arizona - Nevada / California state line follows the Colorado River. I have already given examples of how that makes it necessary to travel through two states to get between points in Arizona. That is definitely one of the oddities in the West.

There is a stream in North America (called North Two Ocean Creek), which splits into two individual streams, one of which eventually makes its way to the Pacific Ocean, and the other eventually reaching the Atlantic (specifically, the Gulf of Mexico.) For clarity, I shall copy and paste the details from the Wikipedia article here:
Two Ocean Pass is a mountain pass on North America's Continental Divide, in the Teton Wilderness, which is part of Wyoming's Bridger-Teton National Forest. The pass is notable for Parting of the Waters, where one stream, North Two Ocean Creek, splits into two distributaries, Pacific Creek and Atlantic Creek, at Parting of the Waters National Natural Landmark. These two creeks ultimately flow into their respective oceans. Atlantic Creek water eventually flows into the Yellowstone River and empties into the Gulf of Mexico via the Missouri River and Mississippi River. Pacific Creek water eventually flows into the Snake River and empties into the Pacific via the Columbia River.

Than there is Triple Divide Peak, Montana. Runoff on the southwestern side of Triple Divide Peak flows west into the Pacific Ocean. Runoff on the northern slope of the mountain flows northeast into the Arctic Ocean through Hudson Bay. Runoff on the southeastern slope of the mountain flows southeast into the Atlantic Ocean through the Gulf of Mexico.

Little Diomede Island, Alaska, (pop. 82) is closer to Russia than it is to the Alaskan mainland. Nearby Big Diomede Island, which belongs to Russia, is just 2.4 miles away, while Little Diomede is about 25 miles west of the Alaskan mainland. Situated in the frigid Bering Sea, Little Diomede is currently accessible primarily by helicopter, weather permitting.
Incidentally, the two Diomede islands are separated by the International Date Line and are roughly a day apart.

Brookings, Oregon to Fields, Oregon, the shortest highway route would first take you into California, then back into Oregon, and finally into Nevada before turning back north into Oregon.

Evanston, Wyoming to Jackson, Wyoming would take you into Utah, back into Wyoming, then slightly into Idaho before crossing back into Wyoming.

Littlefield, Arizona to Yuma, Arizona would take you through Nevada and California.

That's about all I can find.
To go from Las Vegas to Mexican Hat, UT, you have to go through AZ and UT three times.

But that's a interstate trip, not an instate trip. A better example would be St. George, UT to Mexican Hat, UT. It also requires crossing into Arizona two times. But that's still just two states, not three. A lot of trips along state lines in the west have situations like that. But not many require travel through two states to get to a destination in the same state.

Sorry I wasn't following any rules, but thought it was interesting nonetheless.

At about 207 square miles, Isle Royale, Michigan, is the largest island located within an American lake (Lake Superior). There are a few lakes on Isle Royale, one of which (Siskiwit Lake, at 4,150 acres) itself contains a small island called Ryan Island. This is a somewhat uncommon example of “an island within a lake within an island within a lake.”

The exact interchange when Interstate 69 switches between being a north-south highway and being an east-west highway is when it meets fully east-west highway, Interstate 96.
