My card didn't have a sticker. There's been no consistency since vaccines were rolled out and the federal government left it up to the states to figure out how to implement it, with very little useful plan of action.
Mine has a stamp that says DOH-LAKE for the first "Healthcare Professional or Clinic Site" entry, with everything else written by hand (lot number, manufacturer, date of vaccine) for the first line. For the 2nd dose, even the DOH-LAKE was written by hand.
On the back is the reminder that my 2nd dose was scheduled for 4/20 - hand-written, no time specified, and no place to put a specified time. However, they TOLD me what time when they wrote down that date, and I got a reminder in my e-mail of the appointment as well.
