So what do you mean there is no control? They can carve out a cohort and do a negative control which is what they did.

Observational studies don’t start off with a control like control-studies do. However, the CDC used these types of studies to justify masking.

Population density does not correlate with death rate.

There are many potentially confounding presets. For instance the specific time frame and location picked.
And then the fact that many if not most more critically ill rural patients get transferred to the big city as they deteriorate.

All observational studies have many confounding biases, and you can never adjust for all of them. Didn't stop the CDC using observational/ecologic studies (and indeed doing their own) as evidence for mask efficacy.

In fact, now the morons at the CDC are advocating masking for monkeypox.

Mask efficacy will be all over the place based on many variables.
Time frame and location during the Pandemic, along with current dominant variant.
Risk factors of the population, including density, interactivity and medical status.
Common sense dictates that people pay attention to timely local trends, and of course their own risk factors.
