There are some military officers with a similiar attitude she has. It's commonly called the little man condition. An officer who's not sure of himself will sometimes throw his rank around to try to assert authority over others. That's exactly what Boxer was trying to do to him. An officer that is confident in their position and leadership skills does not need to throw his/her rank around unless it is absolutely necessary.

Being addressed as madam is rather diminutive and a bit patronizing in this day and age.
In our military services you learn that the word ma'am is the official way to speak to a superior woman. That man gave her what they use to show deference to female officers and nothing more. He should have avoided any question she may have put to him after her upbraiding of him. The fact that she is a Dem Senator means nothing.
