IIRC some European countries who have a technically "official" state church have, for mostly historical reasons, a "church tax" which is imposed by default on its citizens but which you can opt out of. I'm thinking maybe Denmark or the Netherlands but don't recall for sure. In such countries, you are by default, Lutheran or Dutch Reformed or whatever. It is an assumption.

No. It's not on our IDs. Might as well be, though...since most Americans are born into something, and are raised in it, at least nominally.
