I think they may have more than one that they can choose from, then build as the situation changes. The first one or two may be set in stone, but after that, I think they need some room for flexibility. Not to lead things though.
While I don't think they make up challenges at the last minute I do think they give themselves options so they can head in the direction they want to lead things.

As I said a couple weeks ago, I am wondering if the lack of any air time for Joe, up til this week, meant he was either going to be med-evac'd out, OR, that he was going to come on strong at the end. As I mentioned, I remember a kid from awhile back in that scenario, though I can't remember if he won. Did someone say there WILL be another medical evac? I sure hope not.
I often wonder whether the challenges are manipulated toward the end. Reason being, it always seems like there is one player in a do-or-die situation at the immunity challenge, and that player always seems to win that IC. Maybe they've never even won one before, but they manage to win that one. I don't know....coincidence?

I'm pretty sure they have all the challenges planned out ahead of time. They don't create challenges on the fly to benefit certain players.
No, but they might time them differently depending on how they hoped to skew the results. The most obvious example would be when a strong group of men are more or less owning all the challenges, pagonging the women out of the game. Producers have very long experience with what types of player do best in each kind of challenge. They also have experience building everything (I suspect that some challenge sets get built before the season kicks off, that there's a no go zone somewhere near the camps). I would absolutely not put it past the producers to cherrypick challenges for certain outcome likelihoods.
