Observation: Containers with ice cream toppings not labeled as to contents.
- PHF/TCS food that is cooked and cooled on premises is reheated within 2 hours to 165°F or above for 15 seconds for hot holding
Observation: Person in charge present could not explain the proper way to cool and reheat.
- Packaged foods shall comply with standard of identity requirements
Observation: Packaged Candy not labeled. Label information shall include: (1) The common name of the FOOD, or absent a common name, an adequately descriptive identity statement
- Cooked PHF/TCS food cooled from 135°F to 70°F within 2 hours and from 135°F to 41°F or below in 6 hours
Observation: Person in Charge present could not explain the proper way to cool soup.

Observation: Clean plastic containers stored in a bin on the floor behind the counter.

Observation: No thermometer to check food temperatures.

Observation: Bag-in-a-box syrup stored on the floor in the basement.

Observation: Food prep area and storage of to go containers and cups under sewer lines that are not shielded to intercept potential drips.
- Handwashing signage
Observation: Handwash sign behind the counter is in between the 4 compartment and the handwash - unable to determine which sink should be used. No handwash sign in the Men's restroom.
- Eating, drinking, or using tobacco
Observation: Upon entering the Person In Charge was eating something behind the counter - Corrected on site.
- Demonstration of Knowledge
Observation: Person in Charge at the time of inspection did not have a thermometer to check food temps nor did she know what the temperatures were suppose to be. 2 of the 3 employees did not know anything about a sickness policy or could not provide documentation - Left Form 1-B
- Restriction-presence and use
Observation: Raid not approved for use in a food establishment.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: Missing thermometer in the cooler connected to the ice cream freezer - Corrected on site.

Need to post an Iowa Smokefree Air Act No Smoking sign at the entrance and exit.

Bulk Candy must have the label available to the Public that contains the following information. 1) Product Name. 2) A list of ingredients in order of predominance (by weight). If the product has a standard of identity in the Code of Fedeeral Regulations, it must conform to that standard. 3) Name and address of their manufacturer, packer or distributor. 4) net weight or volume.
