- KDA_3_10111 - --There were packages of Vigo Broccoli Cheddar Mix, Bob's Red Mill Quick Oats, pancake batter (large bag), individual bags of baking chips/nuts/rice on the dry food shelf in the kitchen and in the cabinets in the kitchen that were chewed through or had fresh mouse droppings on and inside the packages. COS- PIC voluntarily discarded. Due to the harborage area in the back storage room several items were difficult to get to and therefore could not be determined if any was adulterated.
- KDA_3_50118A2 - --There was an opened half gallon of milk, an opened half gallon of 1/2 and 1/2 and two opened containers of almond milk in the reach in cooler at the front without any date marking. PIC stated the 1/2 and 1/2 and the milk were opened yesterday 1/6/16 over 24 hours prior. She stated the almond milks had been opened Tuesday 1/5/16. COS- PIC correctly date marked all before mentioned items. There were 3 containers of RTE quiche and two trays of RTE bierocks in the display cooler without any date marking. PIC stated the bierocks were made yesterday 1/6/16 over 24 hours prior and the quiche was made Monday 1/4/16. COS-PIC correctly date marked.
- KDA_4_60111A - --There is fresh mouse droppings (40-50) along the food contact surfaces in the back prep area/kitchen, in the dry goods area and on the front serving line.
- KDA_4_60211E4 - --There is mold build up along the upper inside surface and along the inside pump of the bulk ice unit on the front line.
- KDA_4_90311B - --There were sleeves of single service cups and plates stored directly on the ground in the back storage area.
- KDA_5_20515B - --There is leaking tubes coming off of the espresso unit on the front counter.
- KDA_6_20215A3 - --There is a 1/2 inch gap in the lower left hand corner of the back exit door large enough to allow the entry of pests.
- KDA_6_50111 - --There are ceiling tiles missing above the mop sink where insulation and wiring are exposed. There are holes in the wall in the back storage room area and in the mop sink area. There is tiling missing around the floor drain next to the ice unit at the front counter where standing water is accumulating.
- KDA_6_501111A - --There are hundreds of fresh rodent droppings throughout the facility including areas such as the front serving counter, the back prep counter, the cabinets above the prep counter holding dry goods, in the shelving holding dry goods in the prep area and in the dry goods closet area. There is evidence of nesting in the back storage area (chewed paper/cardboard product). PIC stated one mouse has been caught and he had seen live mice in the facility so he added additional sticky traps. PIC stated there is a pest control company contracted, no paperwork was provided.
- KDA_6_501111B4 - --The back storage room used for dry goods storage is stacked tightly into a small area along with random items creating a harborage area for pests.
- KDA_6_50112A - --There is standing water and mold build up in the missing tile area next to the floor drain at the front counter. There is dirt, grime and mold build up along the inside parameter of the entire facility. There are hundreds of fresh rodent droppings in various areas throughout the facility including countertops in the front and back of the facility, in the cabinets and on the shelves holding dry goods and along the ground all throughout.
- KDA_7_20111A - --There were two cans of glass cleaner stored directly next to a sleeve of single service plates in the back storage area. COS- PIC moved the chemicals to a different area.
- KDA_8_40411 - --There are hundreds of fresh rodent dropping on and inside dry goods in the kitchen/prep area. There are several fresh rodent droppings on the food contact surfaces including the prep counter in the kitchen and the counter at the front service line. There is evidence of nesting and harborage in the back storage area. The facility will be voluntarily closing until health hazard is resolved.
