KDA_3_30412A - --Basket with no handle was used as a scoop for the shredded lettuce and was stored down in direct contact with lettuce. KDA_3_30412B - --Handle of scoop in sugar bin was stored down in direct contact with sugar. Handle of ice scoop used in shaved ice was stored down in direct contact with ice. COS-moved scoops KDA_3_30414B1 - --Wet wiping cloths stored on counters throughout kitchen and not in sanitizer water. KDA_3_30511A1 - --WIC,containers of pineapple juice and cut melons stored directly on the floor and in kitchen bins of rice and beans were stored directly on the floor. KDA_3_50116A1 - --Green peppers stored in pan next to fryer at 83.3. Peppers are cooked and held until ordered and then put back in the fryer before service. Peppers were cooked about an hour ago, per cook. COS-moved to cool rapidly KDA_3_50116A2 - --Display cooler at front counter, flan at 67.6. PIC stated that the cooler had went out this morning. Their ambient air thermometer was reading approximately 62. COS-discarded all potentially hazardous food's in RIC. KDA_3_50118A1 - --Walk in cooler(WIC), spicy pork dated with a made date of 10/12/15. PIC stated that is was not made on the 12th it must be dated wrong, cook stated it was made on Friday(19th). COS-dated KDA_4_10111A - --Blue plastic container used to store shaved ice is not made of food grade material. KDA_4_30111 - --Display cooler at front counter had an ambient temperature of 62. PIC stated it went out this morning. Do not store any PHF in cooler until it has been serviced and is holding food temps at 41 or below KDA_4_60111A - --Blade of both can openers have buildup of food debris. Sticker residue on containers that are stored as clean. COS-cleaned can openers. Containers with stickers need to be pulled and recleaned KDA_5_20411A - --There is not a handsink at the front counter area where drinks are made and a make table is used to put toppings on pork skins. COS-designated sink that was being used to store utensils as a hand sink. KDA_5_40211 - --Cilantro in a colander was sitting down in the bottom of sink that has a direct connection to the sewer. PIC stated that they wash all their fruits and vegetable in that sink. KDA_6_20211A - --No protective covers on lights above 3 vat and drink mix area.
