Food Facility Person in Charge does not has knowledge to how to exclude or restrict ill employee as required by the PA Food Code. 46.111(b)(c) - Duty to Report Disease or Medical Condition: PIC & Food Employee (b) Responsibility of a person in charge to report information to Department. The person in charge shall notify the Department that a food employee is diagnosed with an illness due to Salmonella typhi, Shigella spp., Shiga toxin-producing Escherichia coli or hepatitis A virus. (c) Responsibilities of a food employee or food employee applicant. A food employee or a person who applies for a job as a food employee shall report to the person in charge the information specified in this section in a manner that allows the person in charge to prevent the likelihood of foodborne disease transmission. This information includes the date of onset of jaundice or the date of onset of any illness or condition specified in § 46.112.

Food faciliy does not have a Health Awareness Reporting Procedure for food employees. 46.111(a) - Duty to Report Disease or Medical Condition: Food Facility Operator (a) Responsibility of a food facility operator to require reporting. A food facility operator shall require a food employee and a food employee applicant to whom a conditional offer of employment is made to report to the person in charge information about their health and activities as they relate to diseases described in § 46.112 (relating to diseases or medical conditions that must be reported) or diseases that are transmissible through food.

Defective refrigerator observed at 48-50F. Food items as milk, ham, pizza lasagna diner tray, salad dressing observed between 47-55F. All perishable food stored under improper food storage temperature were discarded while present.

Food facility lacks sanitizer test strips to monitor sanitizer concentration.
- Maintaining & Using a Handwashing Facility (46.825(a) & 46.981(h))
The handwash sink in the prep area was blocked by refill water bottle and containers and not accesible at all times for employee use. Disposable paper was not available at the handwash sink in toilet rooms. No sign or poster posted at the handwash sink in toilet rooms to remind food employees to wash their hands.

Visible physical evidence of bed bugs, mouse and spider activity/infestation observed in the basement storage area and residents bedrooms floors.
