• 2930 - Outer opening of the food establishment is not protected against entry of insects and rodents. All four doors to outside are lacking adequate thresholds.Each door needs to be fitted in some way to seal off pest entry.
  Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
• 3270 - Methods are not being used to control pests. Due to large openings at all doors, particulary at side door in dining room, lizards are entering building at this point. Openings can create entry for mice/insects as well.
  When pests are found use methods to such as trapping devices or other means of pest control. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. See above violation which addresses pest control also.

May 04, 2006 (Routine)

• 0770 - Corrected During Inspection Frozen hamburger patties in the upright GE Freezer appear to be freezer burned. They are stored out of original package.
  These patties should be discarded.
• 2810 - The floors in the dish washing room and the cooler room have flaking surfaces of paint.
  The floors need to be smooth and easily cleanable. The floor surfaces need to be re-painted or resurfaced.
• 2890 - The overhead light fixture in the cooler/freezer room, is without a shield.
  In order to prevent light bulbs from shattering over food items, a protective sheild needs to be mounted on the light fixture.

February 03, 2006 (Routine)

• 0240 - Food workers were not wearing hair restraints.
  Be sure all personell wear either hair nets or caps.
• 0550 - Ice scoop was stored in ice bin with handle down in ice.
  Store scoop outside of ice bin or store in ice with handle up out of ice.
• 2890 - Overhead light fixture in cleaning supply closet does not have a shield.
  A shield needs to be installed to prevent potential shattered bulb pieces from falling down on utensils or equipment.

November 16, 2005 (Routine)

• 1770 B - Observed accumulations of encrusted grease deposits or other soil on the following food contact surfaces: The ice machine has calcium build up on the side walls of the ice bin.
  Clean and sanitize these surfaces for food contact.
• 1770 C - Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: dishwashing trays have calcium and mineral build up..
  Soak in solution to remove the build up andsanitize. Maintain nonfood-contact surfaces of equipment clean.
• 2190 - Repeat Water from the handwashing sink in kitchen was measured at a temperature less than 110F. Measured temperature was 95 deg. F.
  Make necessary adjustments to valves and lines serving the handsink to provide water of at least 110F to allow more effective removal of fatty soils encountered in kitchens.

August 26, 2005 (Routine)

• 1570 - Walk in freezer has frozen condensation at top of unit.
  Have unit serviced.
• 1690 - The pressure gauge for the high temperature warewash machine was below 15 PSI. Measured at 5 psi.
  Rinse dishes twice to insure full sanitization until unit can be fixed. The range for proper operation is between 15 and 25 PSI. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.

May 03, 2005 (Routine)

• 1570 - Rinse cycle on dishwasher not to manufactors requirements. Cycle 10 sec. 15 sec required.
  Have rinse cycle timer adjusted.
• 2190 - Water from the handwashing sink at kitchen was measured at a temperature less than 110F.
  Make necessary adjustments to valves and lines serving the handsink to provide water of at least 110F to allow more effective removal of fatty soils encountered in kitchens.
• 3080 - Less than 50 foot candles of light was noted in the Cooking, washing and food prep areas 20 to 25 fc
  Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor

• 0960 2 - Corrected During Inspection The food contact surface of the butter brush is not durable, nonabsorbent, easily cleanable, resistant to pitting.
  Discarded during the inspection.
• 3260 - Employees are not using a common area to store personal items.
  Employees should have an area just for the storage of personal items. Provide an area.

August 26, 2004 (Routine)

• 3080 - Less than 20 foot candles of light was noted in the Walkin refer and freezer both have bulbs out.
  Provide at least 20 foot candles at a distance of 30 inches from the floor in consumer self-service areas, inside equipment, warewashing, equipment storage, toilet rooms
• 1570 - Hole in ceiling in stockroom.
  Repair the hole to prevent contamination of food from dust and insulation.
• 2890 - Repeat Light bulb in stockroom have missing end caps on 8 bulbs.
  Replace or secure the end caps to the shields.

May 14, 2004 (Routine)

• 0960 1 - Critical The food contact surface of the mixing utensils is not safe.
  Repair or replace the mixing utensils to permit easy cleaning and prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food. Corrected by removing from service damaged utensils.
• 2000 - Single service items observed unprotected from contamination. Drink cups.
  Store single service items in its original protective packaging or inverted in an approved dispenser.
• 2890 - Repeat Light bulb in stockroom not shielded, coated, or otherwise shatter-resistent.
  Shield or replace light bulb with a coated or shatter-resistant bulb.
• 0480 - Unlabeled food containers.
  Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
• 1180 - The food temperature measuring devices (degrees F)are not accurate.
  Provide a temperature measuring device that is accurate within +/- 2 degrees Fahrenheit in the intended range of use.

January 15, 2004 (Routine)

• 0480 - Leftover foods out of original container are stored in Delfield Cooler.
  These foods will be labled /dated by operator.
• 2810 - Floors in storage,cooler and dishwasher rooms are flaking paint. Ceilings in in cleaning supply room is flaking paint also.
  These surfaces need to be painted so they can be kept smooth/cleanable.
• 2890 - Overhead light bulbs aren't shielded in cleaning storage room and closet.
  Each light fixture needs to be shielded.
• 2930 - The ceiling in the corner of the rear storage room(over canned goods) has an opening due to leak damage,repair.
  This opening needs to be sealed off to prevent debri from falling on canned goods as well as prevention of insect/rodent entry.
