- Critical: FOOD CONTACT SURFACES - CROSS CONTAMINATION
Observation: The four compartment sink is currently set up as follows: sink #1: wash, sink #2: rinse, sink #3: sanitize, sink #4: dump sink for bar glasses/straws/ice. The close proximity of the sanitize sink and dump sink create the potential for cross contamination of clean dishes and glassware from used ice/drinks.
Correction: Alter the 4 compartment sink to reduce the risk of contamination. Wash, rinse and sanitize all sinks first, then set up sinks and maintain as follows: sink #1: dump sink for glasses/ice/straws, sink #2: wash, sink #3: rinse, sink #4: sanitize. All dishes must be done in the four compartment sink - do not use the handwashing sink in the kitchen. Correct By: 18-Apr-2014

Observation: A direct connection exists between the sewage system and the drain from the ice machine.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the ice machine drain. Remove the direct connection by implementing an air gap between the two systems. Correct By: 10-May-2014
- Critical: TOXIC SUBSTANCES - MEDICINE - REFRIGERATED STORAGE
Observation: Employee medicine is stored on a shelf above drinks and coffee creamers in the True Pepsi 2 door sliding reach-in cooler and has the potential to contaminate food and/or drinks used in the facility.
Correction: Store employee medicine in a covered leak proof container. Do not store above food/drink items used in the facility. Correct By: 19-Apr-2014
- FOOD PREPARATION SINK
Observation: When tomatoes or lettuce is used in the facility, it is being washed in the handwashing sink located in the kitchen. Very little food preparation is occuring in this facility.
Correction: Do not use a handwashing sink for anything other than handwashing. Use a colander to wash tomato or head of lettuce over the sanitizer sink of the four compartment sink. Wash, rinse and sanitize sink before using for dishes. Correct By: 18-Apr-2014

Observation: There is no service sink provided in food establishment for dumping dirty mop water.

Observation: The female bathroom is not provided with a covered waste receptacle.
