An employee was observed not washing hands before putting on gloves, then handling ready to eat foods An employee was observed using bare hand contact with ready to eat foods while preparing salad in the kitchen. A mop was found stored in the hand sink in the back area of the kitchen, next to the three compartment sink. The hand sink in the back area of the kitchen did not have soap stocked at it, at the time of inspection. A bag of onions were found stored on the floor in the dry storage area. The storage shelves under the front counter of the food cooking line were found to have paint chipping. The door gasket on the cold table in the front cooking line was found torn. The microwave on the front cooking line was found soiled. The shelving above the 3 compartment sink was found soiled. The nacho cheese and chili dispensers were found soiled. The shelf in the front cooking line that that holds the microwave was found soiled. The floor in the walk in cooler was found with buildup of condensate ice, from the condenser unit. The staff was observed not properly washing and rinsing food equipment before placing into the sanitizer solution. Single service cup lids were found stored on the floor in the dry storage area. The wall near the door in the dry storage area was found with a hole in it. In the dry storage area tiles were found with holes in them, thus not allowing the area to be smooth and easily cleanable. The back wall in the area of the three compartment sink was found soiled. The ceiling tiles throughout the food preparation area were found not properly installed or placed properly in the tracking to make the ceiling smooth and easily cleanable.
