Soap and paper towels were not readily available at the warewashing area handsink. Soap was not readily available at the front counter handsink.
Two chemical spray bottle under the front counter handsink were unlabeled as to their contents.
Ice build-up was present in both ice cream reach-in freezers.
The handsink in the womens restroom was cracked.
Several floor tiles throughout the facility were cracked and chipped. The floor under the 3-compartment sink was soiled with debris.
Both men and women restrooms were soiled with debris and accumulated grime.
