Raw beef on the bottom shelf of the meat preparation table was 48�°F.
A spray nozzle was located downstream of the atmospheric vacuum breaker at the mopsink.
A can of WD 40 was stored next to spices on the shelf above the hot water heater.
Food was stored on the floor in the walk in cooler. Meat was improperly thawing on the floor outside of the walk in cooler.
The meat preparation cutting boards were deeply grooved.
Foil being used to line the shelves above the hot water heater was not smooth and easily cleanable. The caulking on the food preparation sink was torn.
The caulking on the three compartment sink was moldy.

The condenser pipe for the walk in cooler drained into a five gallon bucket.
The base coving throughout the meat preparation area was in disrepair. A large hole was present in the wall near the two door reach in cooler. The floor seal in the meat preparation area was worn. The floor in the walk in cooler was soiled with blood.
A gap was present under the back door large enough for pest entrance into the facility.
