Several door gaskets on reach-in coolers and reach-in freezers throughout the facility were torn.
The blade on the table mounted can opener was soiled with food debris.
The storage racks in the warewashing area were severely soiled with food debris.
The concentration of the sanitizer bucket below the pie rack was less than 100 ppm quaternary ammonia .
