Violation descriptions and comments

Several employees were observed putting on gloves without first washing their hands.
An uncovered employee beverage was stored above the mixer near the large donut machine.
Adequate backflow protection was not provided to the faucet near the large donut machine. A shutoff valve and spray nozzle were located downstream of the atmospheric vacuum breaker on the mopsink faucet.
A handcart dolly was stored directly in front of the warewashing area handsink making it inaccessible for handwashing.
Chemical spray bottles near the 3-compartment sink and in the chemical storage area were unlabeled as to their contents.
The storage rack above the 3-compartment sink was not secure. The storage rack above the 3-compartment sink was rusted. The raw wood storage shelves in the chemical storage area were not smooth and easily cleanable.
The exteriors of the bulk bins throughout the facility were soiled with food debris. The top of the ice machine was soiled with dust.
The scoop on top of the ice machine was stored in contact with accumulated dust.
The faucet on the front counter handsink was leaking. The caulking on the drive-thru window handsink was torn.
The lids to the dumpsters were open.
The floor by the large donut machine was soiled with debris. The floor by the 3-compartment sink was soiled with debris. The floor throughout the dry storage area was severely soiled with food debris. The floor edges throughout the facility were soiled with debris. The floor by the ice machine was soiled with debris.
Several lights throughout the facility were not functioning.
A gap greater than 1/4 of an inch was observed between the floor and the back door. Several pieces of unused equipment were stored in the facility.
