Violation descriptions and comments

Raw lamb was stored hanging next to customer food in the walk-in refrigeration.
An employee was observed handling raw meat and then moving on to other tasks without first washing their hands with soap and water.
The preparation hand sink was used for purposes other than hand washing as evidenced by a scrub brush in the hand sink basin. An employee was observed using the preparation area hand sink to fill a mop bucket.
The person in charge was only sanitizing surfaces to prepare raw meat once a day. The person in charge was not sanitizing surfaces used to prepare raw meat when changing between meats of different minimum cooking temperatures. The person in charge was not sanitizing dishes properly. Lack of food safety knowledge was demonstrated by ten critical violations.
Dishes were not being adequately sanitized in the manual ware-washing process as evidenced by no approved sanitizer in the facility.
The three-compartment sink was not supplied with hot and cold water.
A shut-off valve was located downstream from the atmospheric vacuum breaker on the mop sink.
The preparation area hand sink was not equipped with soap. The employee restroom was not equipped with paper towels.
An unlabeled chemical spray bottle was stored above the preparation sink.
The concentration of sanitizer in the spray bottle was greater than 200ppm chlorine.
The preparation sink was badly soiled with grime. The cutting equipment and cutting boards were soiled badly with built-up raw meat debris and juice(blood).
Excessive ice build-up was present in the walk-in freezer and on the door frame of the walk-in freezer.
The fan guards in the walk-in refrigerator were soiled with dust.
The preparation sink was drained to the floor and not the floor drain.
The restroom was not equipped with operational lighting.
The facility's only toilet had a broken bowl, was not operational and holding sewage.

The facility did not have sanitizer for sanitizing their dishes.
Paper towels were not provided at any of the hand sinks in the facility.

The concentration of sanitizer in the sanitizer bucket was greater than 200ppm chlorine.
