All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Walk-in cooler door gasket in poor repair, must fix/replace.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Floors along wallbase in dry storage room, some floor drains, floors under front serving line (hot line), employee room-must be free of litter, food debris.
The walls and ceilings shall be in good repair and easily cleaned. Missing ceiling tiles by cold storage area at employee room must be replaced.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED OVER 20 LIVE SMALL FLIES ON WALLS AT LOCKER ROOM AND AT FRONT PREP HOT LINE. MUST REMOVE ALL PEST ACTIVITY BY DATE OF REINSPECTION (09/13/11).

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. DISHWASH 3-COMPARTMENT SINK FAUCET LEAKS(RIGHT SIDE FAUCET) AND 3-COMPARTMENT MIDDLE COMPARTMENT SINK LEAKING(BY ICE MACHINE). MUST REPAIR AND MAINTAIN SINKS.
INTERIOR PANEL OF ICE MACHINE IN CATERING KITCHEN, SOME DOOR GASKETS IN MAIN DELI KITCHEN, & FILTERS AT HOOD OF COOKING EQUIPMENT IN MAIN DELI KITCHEN MUST BE FREE OF FOOD DEBRIS, DIRT OR DUST.
CUTTING BOARDS WITH DEEP, DARK GROOVES MUST BE REPLACED. GREASE TRAPS IN MAIN KITCHEN HAVE EVIDENCE OF CORROSION ON THE BASES, MUST REPLACE. BOTTOM DOOR GASKET OF SMALL COOLER AT FRONT PREP/SERVING STATION (MAIN DELI KITCHEN) IS RIPPED, MUST BE REPLACED
POOR FLOOR TILES GROUTING AROUND GREASE TRAPS IN MAIN DELI KITCHEN, MUST FIX. WALK-IN COOLER FLOOR ALONG WALL BASES (IN CATERING KITCHEN), MUST BE KEPT CLEAN. THE METAL DOOR PLATE INSIDE WALK-IN COOLER (CATERING KITCHEN) MUST BE FIXED (PULLING AWAY FROM THE FLOOR).
THE CAULKING BETWEEN THE WALLS & SPLASHGUARDS OF THE SINKS OR PREP TABLE WALLS MUST BE REPLACED (EVIDENCE OF MILDEW).
THE METAL PLATE INSIDE THE WALK-IN COOLER DOOR IS PULLING AWAY FROM THE FLOOR, MUST FIX. THE FLOOR DRAIN BEHIND THE ENTRY DOOR MUST BE KEPT CLEAN ALSO.
ALL MILK CRATES USED FOR STORAGE RACKS MUST BE REPLACED WITH REGULAR SHELVINGS, FOR EASY ACCESS TO CLEANING/PEST MONITORING.
MUST PROVIDE LIGHTSHIELD OR SHATTER PROOF BULB AT REACH-IN COLD HOLDING UNIT AT CUSTOMER SELF-SERVICE AREA.
MUST CLEAN AND SANITIZE ICE CHUTE THE FOUNTAIN SODA MACHINE.
MUST RESURFACE FRONT WOODEN PREP TABLES WITH DARK GROOVES.
MUST CLEAN FLOORS AT WALK-IN FREEZER. MUST CLEAN AND REPAIR WALLBASE AT FLOOR IN THE WALK-IN COOLER.
