The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN DEBRIS AROUND HOT WATER TANK.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. STORAGE SHELVES INSIDE REAR COOLER IN REAR NOT CLEAN,MUST CLEAN
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. OBSERVED BUILD-UP DEBRIS IN MOP SINK. MUST CLEAN/MAINTAIN.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. FOUND ICE SCOOP ON TOP OF ICE MACHINE ON TOWEL. MUST PROPERLY STORE ICE SCOOP IN CLEAN SANITIZE BUCKET.

OBSERVED REAR ENTRY DOOR NOT RODENT PROOFED,OPENINGS AT SIDES AND BOTTOM SIDE AREAS OF DOOR. APPROX 1/4 INCH AREAS. MUST REPAIR DOOR TO BE TIGHTFITTING. SERIOUS CITATION ISSUED.
