Found/Observed rodent droppings(20 )in rear storage areas by reach in freezer, also found front entrance, and rear exit doors not rodent proofed, doors had 1/4 to 1/2 inch gaps on bottom of doors, citation issued#H000071327 with a court date of 5/12/2011 at 400 w superior in room #112 at 9:00am.
Found no certified food mgr. with city of chicago food sanitation certificate at time of inspection on 3/16/2011, while preparing, handling potentially hazardous foods(pasta,sauces) at time of inspection on 3/16/2011.Citation issued#H000071327 with a court date of 5/12/2011, at 400 w superior in roo#112 at 9:00am. A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served.
Found food items not properly dated/labeled in reach in freezers, must date and label food items in reach in freezers.
Non food contact surfaces of cooler shelving, storage shelving not clean need detailed cleaning(crevices). Non food contact surfaces of ice machine, cooler door tracks not clean need detailed cleaning, front counter, and bar area storage shelving, reach in cooler shelving need detailed cleaning. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.
Floors under cooking equipment, heavy equipment, 3 compartment sink, and storage shelving not clean need detailed cleaning(corners).Floors in rear storage areas, rear kitchen areas, front counter, and bar areas need detailed cleaning(corners). The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.
Damaged/stained ceiling tiles in kitchen prep areas, rear storage areas, dining areas, and bathrooms shall be replaced. Walls that had peeling paint, holes, gaps in rear kitchen, rear storage areas shall be repair. The walls and ceilings shall be in good repair and easily cleaned.
Light shields in kitchen prep areas not clean need cleaning, missing light shields, broken light shields in rear kitchen, storage areas shall be provided.
Exhaust canopy not clean needs detailed cleaning, exhaust vents(ventilation)in kitchen prep area, coolers, rear storage areas, and bathrooms need cleaning(detailed).
Excess debris and clutter in rear storage areas shall be removed, stock items must be stored six inches off of floor to prevent pest harborage.
