All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INTERIOR ALL COOKS LINE COOLERS WITH FOOD DEBRIS. EXTERIOR ALL COOKS LINE EQUIPMENT WITH EXCESSIVE GREASE. MUST CLEAN ALL AND MAINTAIN.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FLOOR UNDER BOTH BARS WITH DEBRIS AND BEHIND BASEMENT BOX FREEZER. MUST CLEAN AND MAINTAIN. FLOOR THROUGHOUT BASEMENT @ DISH AREA, @ CLEAN DISH STORAGE, THREE COMPARTMENT SINK AND WALK-IN COOLER IN POOR REPAIR WITH CRACKED/MISSING CEMENT, MISSING BASEBOARDS, OPEN CRACKS AND CREVICES. FLOORS THROUGHOUT MUST BE SMOOTH AND EASILY CLEANABLE.
The walls and ceilings shall be in good repair and easily cleaned. HOLE IN WALL AT BASEMENT THREE COMPARTMENT SINK. NO CEILING ABOVE BASEMENT DISH AREA. DAMAGED CEILING ABOVE ICE MACHINE. MUST REPAIR/PROVIDE ALL.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. DIRTY LIGHT SHIELD ABOVE BASEMENT BOX FREEZER. MUST CLEAN. DAMAGED LIGHTING ABOVE BASEMENT THREE COMPARTMENT SINK. MUST REPAIR.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. LEAK AT NOZZLE OF BASEMENT THREE COMPARTMENT SINK. MUST REPAIR.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. ALL FOODS IN WALK-IN COOLER MUST BE STORED IN CLEAN WASHABLE CONTAINERS. NOT USED #10 CANS.
