All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. found rear entrance/exit doors, and garage area doors not rodent proofed, doors had gaps ranging from 1/2 inch, thru 3 inches, shall be repair replaced. above also had no proof of pest control service, must provide proof of service by 6/24/2010. citation issued#H000060181, with court date of 7/22/2010, at 400 w superior in room #112 at 9:00am.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. found observed no certified food mgr. with city of chicago while preparing, handling potentially hazardous foods at time of inspection, citation#H000060181, with court date of 7/22/2010, at 400 w superior in room #112 at 9:00am.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. non food contact surfaces of front counter that had exposed raw wood shall be repair.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. floors under heavy equipment, cooking equipment, not clean need cleaning. floors in bsmt not clean need cleaning.

The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. exhaust vents(ventilation)in prep areas, coolers, and bathrooms need cleaning.
