All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. found rear entrance/exit doors, and garage area doors not rodent proofed, doors had gaps ranging from 1/2 inch, thru 3 inches, shall be repair replaced. above also had no proof of pest control service, must provide proof of service by 6/24/2010. citation issued#H000060181, with court date of 7/22/2010, at 400 w superior in room #112 at 9:00am.
