All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Must replace or repair damaged prep cooler door rubber gasket.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed slight lime build up at interior of ice machine. Must clean and maintain equipment.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Observed accumilated food build up inside floor drains at prep and dishwash area. Instructed manager to clean and maintain interior of all unclean floor drains.
Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. Observed burned out light bulbs above mop sink. Instructed manager to replace with working light bulbs also must provide and install light cover inside walk in cooler.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 1-compartment sink faucet and 3-compartment sink right side compartment pipe leaking. Instructed manager to repair and maintain plumbing.

Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. THE VENTILATION FANS FOR BOTH RESTROOMS ARE NOT OPERATIONAL. MUST REPAIR.
A certified food service manager must be present in all establishments at which potentially hazardous food is prepared or served. NO CERTIFIED FOOD MANAGER PRESENT WITH A CITY SANITATION CERTIFICATE WHILE POTENTIALLY HAZARDOUS FOODS (STEAK AT 143F) IS BEING PREPARED AND SERVED. NO CITY SANITATION CERTIFICATES WERE POSTED AT THE ESTABLISHMENT. MANAGEMENT INSTRUCTED TO HAVE A CERTIFIED FOOD MANAGER PRESENT WITH THEIR ORIGINAL CERTIFICATE AT ALL TIMES WHILE POTENTIALLY HAZARDOUS FOOD IS PREPARED OR SERVED. SERIOUS VIOLATION 7-38-012.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE FOOD DEBRIS ON THE WIRE RACKS IN THE WALK-IN COOLER.

THE VENTILATION FAN IN THE MEN'S RESTROOM IS NOT OPERATIONAL. MUST REPAIR. SERIOUS VIOLATION 7-42-090.
REPAIR THE LEAKING SPRAY NOZZLE AT THE 3 COMPARTMENT SINK.
