All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. 20 smallflys were found in the front bar area. Remove all evidence of insect activity.
All food should be properly protected from contamination during storage, preparation, display, service, and transportation. Glass bottles of beer stored in the north ice machine were removed, the ice dumped out and the interior washed, rinsed and sanitized at this time. The west ice machine has a bacterial buildup inside of the ice bin. Wash, rinse and sanitize the ice machine and remove all of the ice prior to restarting. Machine tagged held for inspection until re-inspected by the Chicago Department of Public Health. E-mail a tag removal request to cicero_marc@cdph.org or nelson_gregory@cdph.org when ready for re-inspection. Add covers to the ice bins in both bars to protect ice from contamination.
All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. The front bar three compartment sink leaks onto the bar floor, repair the drain pipe.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. Clean out the clutter in the basement and in the outside storage container for inspection.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Install the drain lines to the soda guns and run them to a drain. The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. One of the deep fryers does not fit under the exhaust hood, remove a fryer or extend the hood to fit the cooking equipment.
The ice machine was sanitized by empire, tag removed. The ice bins now have lids.
No evidence of smallflies at this time.
The bar 3-comp sink drain no longer leaks.
Coke is installing the soda drain lines now. The deep fryer was moved under the exhaust hood.
The basement and outer storage areas are now organized.
THE FAN GUARDS IN THE BASEMENT FOOD COOLER ARE DIRTY; WASH, RINSE AND SANITIZE THE FAN GUARDS.
The walls and ceilings shall be in good repair and easily cleaned. THE WALL PANEL AND BASEBOARD BEHIND THE KITCHEN HANDSINK IS DIRTY, CLEAN IN DETAIL.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPAIR OR REMOVE THE BROKEN CONVECTION OVEN FROM THE KITCHEN. THE BARS ARE DIRTY, GIVE THE BARS A GOOD GENERAL CLEANING.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE BASEMENT FOOD COOLER FLOORS ARE DIRTY AND THE BAR HAS SPILLED SODA BUILDUP IN THE CENTER OF THE FRONT BAR, CLEAN THE FLOORS FROM CORNER TO CORNER.
MUST PROPERLY STORE WASH CLOTHS IN SANITIZING SOLUTION AT ALL TIMES.MUST PROPERLY STORE ICE SCOOPERS IN ICE RECEPTACLES AT ALL TIMES SO AS TO BE PROTECTED FROM CONTAMINATION BETWEEN USE AND STORAGE.
ALL FOOD PREP STAFF MUST WEAR HAIR AND FACIAL HAIR RESTRAINTS AT ALL TIMES.
MUST PROVIDE 3 SINKS STOPPERS AT 3COMP SINK IN THE KITCHEN. MUST REPAIR SLOW LEAK AT BOTH EXPOSED HANDSINKS IN THE KITCHEN.
MUST REPAIR OR REPLACE DAMAGED CEILING TILES IN FRONT OF REAR EXIT DOOR. MUST CLEAN AND REMOVE EXCESS DUST BUILD-UP AT SAME MENTIONED AREA.
MUST DETAIL CLEAN INTERIOR OF COOLER TO REMOVE FOOD DEBRIS FROM BOTTOM SHELVES. ALSO MUST CLEAN EXTERIOR OF 2DR REFRIGERATOR TO REMOVE GREASE BUILD UP AT DOOR HANDLES.
