All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. THE NEWT AND FISH TANKS IN ROOM # 155 ARE FILTY, CLEAN AND MAINTAIN ALL SURFACES THE CHILDREN CAN TOUCH IN SANITARY CONDITION.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. THE FLOORS IN THE HALLWAY CLOSETS AND THE LAUNDRY ROOM INSIDE OF DAYCARE ROOM # 134 ARE DIRTY AND FULL OF DUSTY OLD CLOTHES, CLEAN OUT AND MAINTAIN ROOM IN ORDER. THE BASEBOARDS IN THE LAUNDRY ROOM AND BELOW THE WEST SIDE SINK HAVE HOLES AND GAPS, CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. THE THERMOMETER IN DAYCARE ROOM #150 APPEARS TO BE OFF, CHECK AND REPLACE AS NEEDED.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.MUST REMOVE DUST FROM ALL VENTS THROUGHOUT PREMISES.
