All food should be properly protected from contamination during storage, preparation, display, service, and transportation. Open meats stored uncovered inside of an unsanitary reach in cooler on and under dirty shelving encrusted with food buildup. Three pounds of product valued at $10.00 was volentarilly discarded at this time. Wash, rinse and sanitize the cooler and shelves fromtop to bottom.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority. The violations from Dec 16,2008 are not corrrected and include: (33) All the reach in coolers and freezers,kitchen tables top to bottom and all cooking equipment are encrusted with food and grease and dirty inside and out. (34) The floors throughout the kitchen are filthy. (35) The walls throughout the kitchen are dirty. Wash, rinse and sanitize the above in detail from top to bottom and maintain on a daily basis.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. Dirty food utensils were found in the clean utensil racks above the sink and dirty dishes and utensils were found all over the kitchen. Wash, rinse and sanitize the dishes and utensils before closing for the evening in order to maintain a sanitary kitchen.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. The cutting boards are stained and dirty, Wash, rinse and sanitize the cutting boards.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. Repair the broken hot water faucet at the three compartment sink.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. Store wiping cloths in sanitizing solution between use.
The reach in cooler is now clean.

The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. FOOD SPILLS IN DRY FOOD STORAGE ROOM, BOXES OF FOOD STORED ON FLOOR, INSTRUCTED TO CLEAN FLOOR FLOOR AND KEEP ALL FOOD ITEMS STORED OFF FLOOR BY AT LEAST 6'.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. TOILET ROOM DOOR MISSING A SELF CLOSING DEVICE, INSTRUCTED TO INSTALL.
ALL FOOD PREP STAFF MUST WEAR HAIR RESTRAINTS AT ALL TIMES.
MUST CLEAN INTERIOR OF COOLERS, AND REFRIGERATOR BOTTOM SHELVES TO REMOVE FOOD SPILLS.MUST ALSO REPAINT RUSTY DRY STORAGE RACK SHELVES AND RACKS AT 2DR COOLER.
MUST INSTALL A SPLASH GUARD AT EXPOSED HAND SINK.
MUST PROPERLY STORE WASH CLOTHS IN SANITIZING SOLUTION AT ALL TIMES.
