FOUND INTERIOR OF WALK IN COOLER AT AN IMPROPER TEMPERATURE OF 43.8 DEGREES F. MUST REPAIR COOLER SO THAT IT MAINTAINS 40 DEGREES F AND BELOW. ALSO FOUND FREEZER AT 24 DEGREES F. MUST REPAIR SO THAT IT MAINTAINS 0 DEGREES F. CRITICAL CITATION ISSUED. 7-38-005A.
All cold food shall be stored at a temperature of 40F or less. FOUND POTENTIALLY HAZARDOUS FOODS AT IMPROPER TEMPERATURES. FOUND APPROXIMATELY 120 LBS OF VARIOUS CHEESES,EGGS,GROUND BEEF,CHICKEN BREASTS AND COOKED RICE AT IMPROPER TEMPERATURES RANGING BETWEEN 45.5-47.0 DEGREES F. MUST MAINTAIN POTENTIALLY HAZARDOUS COLD FOODS AT 40 DEGREES F AND BELOW. CRITICAL CITATION ISSUED 7-38-005A.

FOUND WHITE SUBSTANCE ON INTERIOR AND EXTERIOR OF 3RD COMPARTMENT OF PREP 3 COMPARTMENT SINK TO PREVENT LEAKING. MUST REMOVE SUBSTANCE AND PROPERLY REPAIR SINK TO PREVENT LEAKING. MUST PROVIDE STOPPERS FOR BAR 3 COMP SINKS.

All food should be properly protected from contamination during storage, preparation, display, service, and transportation. FOOD (ICE) NOT PROTECTED DURING STORAGE INSIDE OF THE ICE MACHINE. OBSERVED BLACK MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE ICE MACHINE. A TECHNICIAN WAS CALLED ONSITE TO CLEAN AND SERVICE THE ICE MACHINES. SERIOUS VIOLATION 7-38-005A.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OUTER OPENINGS NOT RODENT PROOF. OBSERVED BOTH DOORS TO THE SIDE AND REAR ALLEY WITH 1/2' GAPS ALONG THE BOTTOM OF THE DOORS. ALSO THE FRONT REVOLVING DOOR HAS GAPS WHERE THE RUBBER HAS BEEN TORN. MUST RODENT PROOF ALL OUTER OPENINGS. ALSO OBSERVED 10 LIVE FRUIT FLIES BEHIND BOTH OF THE BARS AND IN THE RESTROOMS. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE OR MINIMIZE THE PEST ACTIVITY. 3 DEAD RODENTS WERE OBSERVED UNDER THE GARBAGE DUMPSTER AND IN THE ALLEYWAY ALONG THE BUILDING. SERIOUS VIOLATION 7-38-020.
A separate and distinct offense shall be deemed to have been committed for each Minor violation that is not corrected upon re-inspection by the health authority.
