PREVIOUS MINOR VIOLATIONS FROM REPORT #156871 ON 6/4/09 NOT CORRECTED: 32/01-MUST REPLACE BROKEN RUBBER GASKET AROUND PREP COOLER DOOR IN THE KITCHEN (NEXT TO STEAM TABLE-ON RIGHT SIDE). MUST REPLACE BROKEN INTERIOR OF CHEST FREEZER DOOR BY OFFICE IN THE BASEMENT. MUST CLEAN, REPAINT OR REPLACE RUSTY METAL RACKS INSIDE THE WALK-IN COOLER. 34/01-NOTED WATER POOLING UNDER AND AROUND THE ICE MACHINE IN THE BASEMENT. KEEP FLOORS DRY AT ALL TIMES. 35/01-MUST REMOVE PEELING PAINT FROM IRON CEILING ABOVE PREP AREA/HALLWAY TO THE KITCHEN. 36/01-MUST REPLACE MELTING LIGHT SHIELDS AND MISSING END CAPS ABOVE KITCHEN. 38/01-LEAKING DRAIN PIPE (MIDDLE COMPARTMENT) OF THE 3 COMP. SINK (REAR). SERIOUS VIOLATION 7-42-090.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPLACE THE PARTICLEBOARD SHELVING IN THE PREP AREA AND REPLACE WITH A WATER RESISTANT MATERIAL. REPLACE THE TORN DOOR GASKETS ON THE INSIDE OF THE SALAD COOLER DOORS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE VENTILATION HOOD AND FILTERS ABOVE THE COOKING EQUIPMENT, INTERIOR OF THE ICE MACHINE, FAN COVER IN THE WALK-IN COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR ALONG THE WALLS IN THE PREP AREA. MAINTAIN THE FLOOR DRY IN THE PREP AREA.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE A LIGHT SHIELD FOR THE FIXTURE ABOVE THE STOVE.
OBSERVED A DRAIN LINE WATER LEAK AT THE EXPOSED HANDSINK IN THE PREP AREA. MUST REPAIR SOURCE OF LEAK.

OBSERVED EXCESSIVE AMOUNT OF CLUTTER IN THE BASEMENT STORAGE ROOMS. MUST REMOVE UNNECESSARY ITEMS AND STORE REMAINING ITEMS CLEAN AND OFF OF THE FLOOR.

Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. LABEL WHITE CONTAINERS UNDER THE PREP TABLE,INSIDE ARE STORED DRY FOOD (FLOUR-SUGAR ETC.)
