All rooms in which food or drink is prepared, or in which utensils are washed, shall be lighted so that a minimum of 50-foot candles of light is available on all work surfaces.OBSERVED DELI FREEZER LIGHT NOT WORKING, AND LIGHT SHIELDS FROZEN INSTRUCTED TO CLEAN. OBSERVED THE FOLLOWING LIGHT SHIELDS NOT CLEAN DELI, AND 2ND FLOOR EMPLOYEES LOUNGE INSTRUCTED TO CLEAN SHIELDS. OBSERVED MISSING LIGHT SHIELD IN ICE CREAM FREEZER INSTRUCTED TO PROVIDE.

MUST CLEAN AND ORGANIZE STORAGE AREAS AND REMOVE UNNECESSARY ARTICLES. ARTICLES MUST BE STORED OFF FLOORS, AWAY FROM WALLS.
All utensils shall be thoroughly cleaned and sanitized after each usage. MUST CLEAN-SANITIZE; DISPLAY COOLERS, FREEZERS, BOTTOM PANELS OF PREP TABLES AT ALL PREP AREAS, CAN OPENER, HOT BOXES, OVENS AND FRYERS AT HOT FOODS PREP AREA, STORAGE CARTS AT BAKERY AND SOME DISPLAY SHELVES THRU-OUT PREMISES.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. EXCESSIVE DEBRIS; MUST CLEAN FLOORS THRU-OUT PREMISES, UNDER, BEHIND EQUIPMENT AT CORNERS AND ALONG WALLS AND INSIDE ALL WALK IN COOLERS.
The walls and ceilings shall be in good repair and easily cleaned. MUST CLEAN FAN GUARDS INSIDE DAIRY WALK IN COOLER.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKING PIPE ABOVE DISH MACHINE.

THE FOLLOWING NOT CLEAN; MUST CLEAN/SANITIZE; WARMERS AND COOLERS IN MAIN KITCHEN; SLICERS AND CAN OPENER AT DELI AREA; AND CABINETS, COUNTERS AND DISH MACHINE AT BAKERY.
MUST CLEAN FILTER ABOVE COOKING EQUIPMENT IN MAIN KITCHEN.
INSTRUCTED TO CLEAN FLOORS, UNDER, BEHIND EQUIPMENT AT CORNERS AND ALONG WALLS AT MAIN KITCHEN, DELI, BAKERY AREA AND THRU-OUT REAR STORAGE AREA. ALSO INSTRUCTED TO CLEAN FLOOR DRAINS UNDER ALL SINKS.
INSTRUCTED TO REMOVE UNNECESSARY ARTICLES, CLEAN AND ORGANIZE PRODUCE PREP AREA TO PREVENT PEST HARBORAGE. ARTICLES MUST BE STORED OFF FLOORS, AWAY FROM WALLS.
