All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Found shelving units not wiped down by hot water dispenser area.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. Found floors not mopped and swept by mop sink areas.

NO CITY OF CHICAGO CERTIFIED FOOD HANDLER ON PREMISES WHILE POTENTIALLY HAZARDOUS FOODS- CHICKEN, MACARONI AND CHEESE,ETC- ARE HANDLED. INSTRUCTED MANAGER A CITY OF CHICAGO CERTIFIED FOOD HANDLER MUST BE ON PREMISES WHILE POTENTIALLY HAZARDOUS FOODS ARE HANDLED.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. OBSERVED FOOD DEBRIS INSIDE PIZZA PREP COOLER. MUST CLEAN AND MAINTAIN COOLER.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PREP AREA HANDWASH BOWL COLD WATER VALVE OFF (BY WALK IN COOLER). MUST TURN COLD WATER VALVE ON.
