Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. The following sinks in need of repair- Bar 3-compartment sink middle pipe leaking, pot wash 3-compartment sink hot water handle leaks and left side compartment pipe leaks at same sink.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. Observed damaged Pizza prep coolers rubber door gaskets. Must repair/replace all damaged gaskets also observed lower side of bar counter surface peeling must repair.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. Observed slight lime build up inside ice machine. Must clean and sanitize/maintain equipment.
The walls and ceilings shall be in good repair and easily cleaned. Observed accumilated dust build up on ceiling tiles in rear prep area by ceiling vent cover. Must clean or replace all unclean tiles.
REPAIR THE SOURCE OF THE WATER LEAK BEHIND THE ICE MACHINE.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED LIVE FRUIT FLIES ON THE WALLS OF THE BAR AREA AND BEVERAGE SERVICE AREA IN THE PREP AREA. APPROXIMATELY 30 LIVE FRUIT FLIES WERE OBSERVED ON THE WALLS AND CEILING OF THE BAR AND PREP AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO MINIMIZE OR ELIMINATE THE PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. STORE THE TASTING SPOONS AT THE HOT LINE INVERTED SO THAT ONLY THE HANDLES ARE TOUCHED TO PREVENT CONTAMINATION.
The walls and ceilings shall be in good repair and easily cleaned. PROVIDE A DOOR SWEEP ALONG THE BOTTOM OF THE DOOR TO THE BASEMENT STORAGE ROOM. DETAIL CLEAN THE WALL BEHIND AND AROUND THE BAR. DETAIL CLEAN THE CEILING TILES AND LIGHT SHIELDS AROUND THE SUPPLY VENTS.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE INTERIOR OF THE PIZZA PREP COOLER, INTERIOR OF STACKABLE OVENS IN THE PREP AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR DRAINS BEHIND THE BAR AND IN THE PREP AREA. DETAIL CLEAN THE FLOOR UNDER THE BAR AND IN THE BASEMENT STORAGE ROOM ESPECIALLY UNDER THE FOUNTAIN POP RACK.
VIOLATION WAS ABATED. NO FRUIT FLY ACTIVITY WAS OBSERVED.

CAULKING BETWEEN THE WALL & SPLASHGUARD IN THE DISHROOM & ALSO BETWEEN THE BASEBOARD & WALL UNDER THE DISHMACHINE MUST BE REPLACED (PEELED-OFF OR MISSING). The walls and ceilings shall be in good repair and easily cleaned.
